Discharge
Planning - If you will need some in-home
assistance following your discharge, our Social Workers
will be happy to assist you. Referrals can be made to home
health agencies, volunteer organizations, Meals on Wheels
and a variety of other community resources.
Patient Representative - A patient representative is your liaison while you are in the hospital and is available to help patients and their families resolve questions and concerns regarding care at Jefferson Memorial Hospital.
If you have concerns or questions about patient care and safety in the hospital that the hospital has not addressed, you may contact the patient representative at extension 1700 from within the hospital or (304) 728-1700 from outside the hospital. If concerns cannot be resolved through the hospital, you may contact the Joint Commission on Accreditation of Healthcare Organizations by calling (800) 994-6610. Financial
Assistance - An Eligibility Worker from
the West Virginia Department of Health and Human Resources
is available at Jefferson Memorial Hospital to take applications
for Medicaid, food stamps, Aid to Families of Dependent
Children, emergency financial aid and other services. Representatives
from the business office can determine if you qualify for
help with your hospital bill. If you are in need of Financial
Aid, contact the Social Services office to schedule an appointment.
Counseling - Professional counseling
referrals can be made for victims of domestic violence,
child or elder abuse, as well as as those experiencing emotinoal
distress associated with personal trauma, grief and situational
problems.
Advance
Directives -The Social Services Department can assist you in the execution
of a living will, medical or durable power of attorney.
Notary
Public-
The Social Services Department, as well as other offices
at Jefferson Memorial Hospital, offers a notarizing service
for those documents you may incur while staying at Jefferson
Memorial Hospital
Nursing
Home Referral - In the event that you will not be able to return home immediately
following your hospital stay, the Social Services Office
can assist with nursing home applications and referrals
to a nursing facility of your choice.
Spiritual
Needs - The Social Services Office can assist you in acquiring a visit
from a clergy member, while you are a patient at Jefferson
Memorial Hospital.
Rehabilitation - Patients who have suffered
a brain injury such as stroke or experienced other physical
trauma may benefit from rehabilitation and the Social Services
Office can make referrals for a wide variety of rehabilitation
services, which are available from accredited rehabilitation
hospitals.
Ethics
Consultation - A social worker can initiate an ethics consultation if you or
your family are having difficulty with medical/ethical decisions
in regards to treatment issues.
Birth
to Three Development Assessment Program - The program
is designed to assess and assist children who may be developing
slowly and is available free of charge to any resident of
Jefferson County.
Skilled Nursing Unit - For a smooth transition from hospital to home.
