Benefits — Reminders
Enrollment Periods
Full-time and part-time status are eligible for most benefits after thirty (30) calendar days of employment. Currently enrolled employees are eligible to change their elections at Open Enrollment. Employees will receive notification of the date and time to meet with a Benefits Enroller to enroll as a new hire or to make changes at Open Enrollment. If an employee does not attend his/her meeting with the Benefits Enroller, the employee is not eligible to sign up for benefits until the next Open Enrollment.
Qualifying Events
If the following qualifying events occur, an employee is eligible to change his/her benefits within thirty (30) days of the event. A qualifying event is a change in legal marital status, number of dependents, or employee's or spouse's employment. The employee is responsible for contacting the Human Resources Department to complete the appropriate documents within thirty (30) calendar days of qualifying event. Appropriate documentation must be provided by the employee to support the request for change (i.e. marriage certificate, birth certification). If the Human Resources Department is not notified and proper forms completed within the thirty (30) calendar days, the employee is not eligible to change his/her benefit elections until the next Open Enrollment.
Name Change
If an employee has a last name change, the employee must update their Social Security card and present the card to the Human Resources Department to have their name changed in the payroll system. Failure to have the correct last name on a Social Security card can result in problems with W-2 information for personal tax filing.
Change in Address
Employees are responsible for maintaining accurate information on file with the Human Resources Department. Inaccurate information can result in misdirection of employees' W-2 or important employer information.
