Self-Management Classes:
"Life With Diabetes"

Description: This
four - week program teaches the person with diabetes self-
management skills. Topics include: What is Diabetes?, the
diabetes care team, resources, family support, feelings and
coping, why control is important, diet and meal planning,
medications, monitoring, complications, exercise, travel,
hygiene including foot and skin care. Our diabetic team consists
of a Medical Physician, three Registered Nurses, a Certified
Diabetic Educator, a Registered Dietitian and a Pharm D. |

Population: All diabetics and family and /or significant
